Covid-19 Pre Course Checklist - Commercial Bookings ONLY
Forth Valley College is committed to ensuring students and staff attending our college campuses are in a COVID-19 secure environment.
We have put in place robust risk assessments to safeguard against the risk of infection and ask that prior to attending your course you are able to answer no to all of the following questions.
Have you had close contact with or cared for someone diagnosed with COVID-19 within the last 14 days?
Have you been asked to self-isolate after returning to the UK within the last 14 days?
Do you have either:
- a high temperature or fever?
- a new continuous cough?
- a loss of or change in sense of smell or taste?
If you answer yes, to any of the above please do not attend your course and contact us immediately to reschedule your course. If you reside in an area currently under government-imposed local COVID-19 lockdown, please contact the College and we will rearrange your course for a date and time when your local restrictions have been eased.
For assessment purposes, practical courses may require candidates and assessors to break 2-meter physical distancing. This has been risk assessed and where practically possible will be avoided. Where this is required it is a mandatory requirement that individuals must wear a face mask.
Forth Valley College is following Scottish Government advice to safeguard all. Any candidates attending college-based courses must do this through their own choice.
Please fill in the form below and submit prior to starting your course.
