Lateral Flow Test Consent Form

I have had the opportunity to consider the information provided to me by Forth Valley College about this testing programme. I have had the opportunity to ask any questions about the programme and, if I have, I have had these answered satisfactorily. For students under the age of 16: I have gained consent from my parents/carers/guardians to take part in this testing and I have provided the name of my parent/carer/guardian below.

I agree that if my test results are confirmed to be positive, I will inform the College to support contact tracing
